Introduction to Gastroenterology Assisting
Gastroenterology deals with the entire digestive tract and related disorders. Medical assistants in this field need to understand both the common diseases and diagnostic procedures used to identify them.
The gastroenterology specialty covers three main categories of disorders: those affecting the esophagus and stomach, intestinal disorders, and conditions of the liver and gallbladder. Each requires specific knowledge and testing approaches.
Diagnostic procedures in gastroenterology often include examining different abdominal quadrants, performing specialized tests like sigmoidoscopy and colonoscopy, conducting proctologic examinations, and screening for occult (hidden) blood in stool samples.
Remember This: The gastrointestinal (GI) system is often called the alimentary canal because it handles three key functions: digesting food, absorbing nutrients, and eliminating waste products.
Gastrointestinal Diseases and Specialists
A gastroenterologist treats diseases of the stomach, intestines and accessory organs, while a proctologist specializes in disorders of the rectum and anus. These specialists help patients whose digestive systems aren't functioning properly.
The GI system works through peristalsis and sphincter muscles (circular muscles that control passage between organs). Common symptoms that bring patients to these specialists include nausea, vomiting, diarrhea, constipation, and abdominal pain.
Esophageal and stomach disorders include hiatal hernia (when the stomach protrudes into another tissue) and reflux (stomach contents flowing backward into the esophagus). Gastroesophageal reflux disease (GERD) occurs when the sphincter muscle doesn't close properly, allowing acidic stomach contents to cause heartburn.
Gastric and duodenal ulcers develop when the protective mucosal membrane breaks down in the stomach or intestines, causing pain and sometimes bleeding. Pyloric stenosis involves the narrowing of the pyloric sphincter at the end of the stomach.
Clinical Insight: Many GI symptoms overlap between different conditions, which is why proper diagnostic testing is essential for accurate treatment planning.
Intestinal and Accessory Organ Disorders
Intestinal problems range from common to complex. Food-borne illness simply means getting sick after eating contaminated food, while irritable bowel syndrome (IBS) causes chronic abdominal cramping, pain, and bloating with either diarrhea or constipation. Acute appendicitis is a painful inflammation of the appendix, sometimes caused by a hardened mass of feces.
More serious intestinal conditions include Crohn's disease (inflammation typically in the ileum with unknown cause, classified as an autoimmune disease), celiac disease (which prevents proper nutrient absorption), and diverticular disease (pouches forming in the colon lining). Hemorrhoids are essentially varicose veins of the anus and rectum.
The liver detoxifies harmful substances while the gallbladder stores bile. Common disorders include cholelithiasis (gallstones formed from cholesterol or bile salts), which might require a cholecystectomy (surgical removal of the gallbladder). Cirrhosis occurs when liver tissue becomes fibrous and hardened, causing liver failure and sometimes jaundice (yellowing of the skin).
Hepatitis is liver inflammation from infection, chemical exposure, or drug complications. Viral hepatitis includes types A through G, with different transmission routes—hepatitis B and C spread through blood and bodily fluids, while types A and E are transmitted through feces.
Important Note: The liver is essential for survival, but the gallbladder can be removed without preventing a person from living a relatively normal life.
